Firms with more than 50 employees are required to offer insurance. Most pay over 50% of the premiums; employees pay the balance. HR departments burn time and money to manage all the problems. These costs spur outsourcing, robots, and Ai to replace employees.
Small business owners face a double-barreled shotgun; they need healthcare for their own families and the families of their workers. They bear higher costs per employee for health insurance premiums and administration.
Our healthcare system is a huge obstacle to job creation and business startups.
The Bureaucracy Burden
Doctors, hospitals and clinics drown in red tape. Most insurance companies do not treat patients; they deny coverage, delay treatment, and pile on paperwork. It is pure bureaucracy. Money down the drain.

We spend 17.5% of GDP on healthcare. Most first-world nations have universal coverage, and they spend 10% to 12.5% of GDP. We can provide great care to all Americans with 16% - 17.5% GDP.
Doctors, nurses, and other healthcare workers should be well paid. To be at their best each day, they need good working conditions. Hospitals and clinics should be profitable and well maintained. We must reward innovation, be it new medicines, techniques or technologies.
Who is Covered?
All American citizens and taxpaying legal residents will be covered. Restrictions will be set to prevent medical tourism.
NOPSWIcare Providers
We will persuade healthcare providers to participate with fair compensation paid in a timely manner with much less red tape.
Providers, public, private, for profit, nonprofit, and religious, will compete to deliver the best care. Veterans can choose VA care or any provider in the system.
NOPWSIcare Payments to Healthcare Providers
Absent a true market, we need accountants, auditors, economists, and medical professionals to determine payment rates. The compensation team will look at costs, margins, supply and demand. Transparency will be vital.
They will lower rates for overpriced specialties, services and medicines. Providers of overpriced specialties, services and medicines will be the chief opponents of NOPWSIcare.
We cannot allow the donor class to run the compensation team. The team must be isolated from lobbyists. Current or former officers of large healthcare firms should not be on the team. Team members may not become lobbyists after they leave the team.
To further protect taxpayers, we will ask patients to review their bills for accuracy. The bills will be simple, but they will include all charges. For patients who are willing to help, we will provide online forms, instructions, definitions, and FAQs.
Big Bureaucracy Cuts!
We will not need health insurance companies and state health insurance regulators. Both are pure bureaucracy.
NOPWSIcare will replace Medicare, the Affordable Care Act, and the healthcare portion of Medicaid with a less complex, more transparent system.
Instead of collecting payments from scores of reluctant insurers, providers will enjoy simple billing with far fewer pre-approval requests. Employers will no longer need to administer healthcare coverage for their employees. The court system will see fewer bankruptcies.
A simple, transparent system will aid congressional oversight. Audits will be easier to perform. Voters, reporters and our elected representatives will be able to follow the money.
NOPWSIcare funding sources:
Funds supporting Medicare, the Affordable Care Act, and the healthcare portion of Medicaid will be shifted to NOPWSIcare. Medicaid will continue to provide long-term care.
Replacing Insurance Premiums
Health insurance premiums will be eliminated. We will need to replace premium revenue with tax revenue.
Corporate Taxes
A corporate tax increase should collect revenue equal to corporate savings from the elimination of healthcare premiums and administration costs.
Income Tax
Most state and local taxes and fees are regressive. The rich pay a lower percentage of their income than the rest of us. Federal tax policy can balance the tax burden, but for now we have the inequality of a banana republic. The 2nd Law of Economics refutes the idea that we must favor the rich to make capitalism work.
Therefore, the wealthy should bear much of the burden of the premium replacement taxes. Rates on large incomes will go up. We may need to broaden the tax base by lowering the standard deduction. Most of us will pay much less in tax increases than we now pay in insurance premiums.
State and Local Governments
State and local governments will not need to fund and administer healthcare coverage for their employees. Public hospitals and clinics will be compensated like other NOPSWIcare providers. They will compete for patients with other providers. State and local subsidies should not be necessary. These savings will compensate for recent cuts in federal funds sent to state and local governments.

If President Trump signs it, NOPWSIcare will become Trumpcare. But why would Donald Trump sign a bill that would increase his own taxes?
President Trump pledged to protect Social Security, Medicare and Medicaid. He could have done better.
The Congressional Budget Office estimates the One Big Beautiful Bill (OBBB) will reduce healthcare funding by $1 trillion over ten years, while adding $3.4 trillion to the deficit.[1] The OBBB cuts will make Obamacare too expensive for millions of Americans. Changes in eligibility rules will kick millions off Medicaid in 2027, after the midterm elections.
The OBBB will increase the net costs of Social Security’s Old Age, Survivors, and Disability Insurance (OASDI) program by $169 billion over the calendar years 2025 - 2034.[2] The money will come out of the OASDI trust fund.
Medicare spending will be cut $491 billion over the years 2027 - 2034 if Congress fails to grant SPAYGO waivers.[3]
It may be unfair, but Trump is receiving more of the blame than congress. To enter the pantheon of populists, Trump needs to go big. National healthcare is the answer.
