I sold health insurance in my younger days, long before Obamacare. I worked with great people and a strong company offering high-quality coverage at competitive rates. It was great but for three problems. I could not sell healthcare coverage to those who needed it the most - people with major illnesses. Second, all preexisting conditions would not be covered for two years. Third, some preexisting conditions could be subject to a rider and not covered at all. These problems took the fun out of the job.

The Burden on Business & Citizens

Firms with over 50 employees are required to offer insurance. Most pay over 50% of the premiums; employees pay the balance. HR departments waste time and money managing all the problems. These costs encourage the use of outsourcing, robots, and Ai to replace employees.

Small business owners face a double-barreled shotgun; they need healthcare for their own families and the families of their workers. They bear higher costs per employee for health insurance premiums and administration.

Our healthcare system is a major obstacle to job creation and business startups.

The Bureaucracy Burden

Doctors, hospitals and clinics drown in red tape. Most insurance companies do not treat patients; they deny coverage, delay treatment, and pile on paperwork. It is pure bureaucracy. Money down the drain. NOPSWIcare will spend less time and money on bureaucracy, red-tape and paperwork and more on healthcare.

Medical Bankruptcy

No insurance? The system is designed to bankrupt you. The ambulance ride puts you in the hole. At the hospital, you pay more, often several times more, than insurers pay. If you can’t pay, you may be sued and/or hounded by debt collectors. Your health care may be delayed or denied.

Even with insurance, Americans struggle to pay deductibles, co-pays, out-of-network charges, uncovered expenses, and denied claims. Medical debt is the #1 cause of personal bankruptcy in America. Health insurance fails its basic purpose; we are not protected from large losses.

Americans deserve great healthcare. We will continue to demand that our employers provide it until we create a system that covers all of us.

Great Healthcare for America

We spend 17.5% of GDP on healthcare. Most first-world nations have universal coverage, and they spend 10% to 12.5% of GDP. We can provide excellent care to all Americans with 15% - 16% of GDP.

Doctors, nurses, and other healthcare workers should be well paid. To be at their best each day, they need good working conditions. Hospitals and clinics should be profitable and well-maintained. We must reward innovation, be it new medicines, techniques, or technologies.

Who is Covered?

NOPWSIcare will cover all American citizens and taxpaying legal residents. Restrictions will be set to prevent medical tourism.

NOPSWIcare Providers

We will persuade healthcare providers to participate with fair compensation paid promptly, with much less red tape.

Providers - public, private, for-profit, nonprofit, and religious - will compete to deliver the best care. Veterans may choose Veterans Affairs (VA) care or any NOPSWIcare provider. Under-used VA facilities may serve the public under NOPSWIcare.

NOPWSIcare Payments to Healthcare Providers

Have you ever price-shopped for hospital care? Emergency care? Absent a true market, NOPSWIcare will use a team of accountants, auditors, economists, and medical professionals to determine payment rates. The compensation team will look at costs, margins, supply and demand. Transparency will be vital.

The compensation team will lower rates for overpriced specialties, services and drugs. Providers of overpriced specialties, services, and drugs will be the chief opponents of NOPWSIcare.

Bad actors try to game every system. A division of the compensation team will investigate fraud. Patients will pay only modest deductibles and co-pays; however, we will ask patients to review their bills for accuracy. Participating patients will be given clear instructions and support. Medical bills will be straightforward and include all charges.

We cannot allow the donor class to run the compensation team. We must isolate the team from lobbyists. Current or former officers of large healthcare firms should not be on the team. Team members cannot become lobbyists after they leave the team.

Competition

Competition between NOPWSIcare providers will be based on the quality of their service. People talk about their healthcare providers.

Big Bureaucracy Cuts!

We will not need health insurance companies and state health insurance regulators. Both are pure bureaucracy.

NOPWSIcare will replace Medicare, the Affordable Care Act, and the healthcare portion of Medicaid with a less complex, more transparent system.
Instead of collecting payments from many reluctant insurers, providers will enjoy streamlined billing with far fewer pre-approval requests. Employers will no longer need to administer healthcare coverage for their employees. The court system will see fewer bankruptcies.

A simple, transparent system will aid congressional oversight. Audits will be easier to perform. Voters, reporters, and our elected representatives will be able to follow the money.

NOPWSIcare funding sources:

NOPWSIcare will receive the funds that now support Medicare, the Affordable Care Act, and the healthcare portion of Medicaid. Medicaid will continue to provide long-term care.

Replacing Insurance Premiums

NOPWSIcare will eliminate health insurance premiums. We will need to replace premium revenue with tax revenue.

Balance the Tax Burden

Most state and local taxes and fees are regressive. The rich pay a lower percentage of their income than the rest of us. Federal tax policy can balance the tax burden, but for now, we have the wealth inequality of a third-world dictatorship. Our article, The 2nd Law of Economics, refutes the idea that we must favor the rich to make capitalism work.

The rich have too much power and money. Our economy is more focused on providing goods and services for the rich and more dependent on the rich buying goods and services they don't need. They buy up existing housing as safe investments, driving up housing prices and jacking up the rents. Vulture capitalists buy up viable businesses and suck them dry. They use a BS excuse called "creative destruction". Private equity can do great good and cause great harm. To get more good and less harm, we must take control of the incentives.

Wealth Tax or Income Tax?

A wealth tax on households with over $20,000,000 in assets would be the fair way to replace premium revenue. The alternative would be a combination of higher income tax rates and a lower standard deduction. This would increase middle-class and working-class taxes, but the elimination of healthcare premiums will offset the tax increase except in the "top 10%" tax brackets.

Graduated Corporate Tax

We can use a graduated corporate income tax to generate revenue equal to the amount publicly held C corporations save from the elimination of healthcare premiums. Unlike previous graduated corporate taxes, the upper tax brackets will create incentives to deter mergers and encourage conglomerates to divide into smaller businesses. The corporations with the largest profits will pay the highest rates. Small and medium sized firms will have a better chance to compete. We will all benefit from increased competition.

Private Companies

Privately owned businesses will also see large savings from the elimination of healthcare premiums and administrative costs. Unlike C corporations, private entities do not pay a corporate income tax. Instead, profits “pass through” to the owners as personal income.

The elimination of the 20% tax break on pass-through income would help replace the premium revenue that is now paid by private employers. The 20% break gives private equity an unfair advantage over C corporations, which pay corporate income tax and whose owners pay capital gains taxes.

To help small businesses and the self-employed we would favor keeping the 20% tax break on the first $50,000 of pass-through income, and giving statutory employees a comparable increase in their standard deduction to place them on equal footing with small business owners and the self-employed.

The Tax Burden

The rich, (over $20,000,000 assets) will pay more in taxes than they now pay in premiums. Small corporations will pay less, medium-sized about the same, and the largest corporations will pay more.

Acquiring and maintaining healthcare coverage will be easier. Changing jobs and hiring new workers will be easier. Starting a family will be easier. Escaping poverty will be easier. Running a business will be easier. Running a small business will be much easier. Starting a business will be much easier.

State and Local Government

Cities, counties and states will not need to fund and administer healthcare coverage for their employees. Public hospitals and clinics will receive NOPSWIcare compensation; they will no longer need state and local funds. These savings will allow cuts in federal funds sent to state and local governments for other purposes.

President Trump has been waiting for congress to pass a healthcare plan better than Obamacare since 2017. NOPSWIcare is better than Obamacare, it is the plan he has been waiting for.

To cut bureaucracy and work transparently, NOPWSIcare will be a single-payer healthcare system (props to those who already guessed). Why single payer? The alternative is multiple payers, hundreds of insurance companies and government agencies that add overhead costs, bureaucracy, and red tape, but no medical services. Inefficiency is the hallmark of our current healthcare system.
